  2. A statement under penalty of perjury that you have a good-faith belief that the material was removed or disabled as a result of a mistake or misidentification.
  3. Your name, address, telephone number, and email address, and a statement that you consent to the jurisdiction of the federal court in your district or, if outside the U.S., the district where we are located.
